- You must be over the age of 50.
- Access to CPOA accommodation is subject to availability and approval by the CPOA.
- You hereby grant the CPOA access to your information as provided on the form.
- The CPOA will treat the information, you provided, as confidential.
- You will be contacted by the CPOA.
